USAF Flyers: PHAQ must be completed on your myIMR prior to booking.
All Flyers and OHE: Must complete audiogram PRIOR to appointment (call 623-856-3111). Bring glasses if RX'd.

Appointment

FLY PHA OR OHE

Annual FLY PHA or Annual OHE. Including Fire or SFS members.

Foreign Aviators: You must first in-process which is different from a FLY PHA. If you are due for your Annual FLY PHA then you can schedule here. Please follow all the rules below, except the PHAQ.
USAF FLY PHA ONLY: Ensure PHAQ is done on your MyIMR.
Everyone: Make sure Audiogram is scheduled and completed prior to this appointment (856-3111). Bring any and all prescribed glasses/contacts. Failure to do so will result in your appointment being canceled.
EOD, SFS and Fire OHE: Recommend you schedule an lab appointment 30min before or 1 hour after OHE slot

OHE 40 Min

Annual OHE Only

1. Log into your IMR to see your requirements. https://asimsimr.health.mil/imr/myimr.aspx
2. Complete your Occupational Health Questionnares on your myIMR,if applicable (i.e. Firefighter questionaire, respirator questionaire, hazard questionnaires, etc).
3. Schedule your audiogram (if applicable) by calling Occupational Health at 623-856-3111
Note: If you are due for any labs, these will become active 24hrs prior to your OHE appointment and remain active for 60 days in total. Please schedue with Lab 30 min before or 1 hour after your scheduled appointment for best care. Your OHE isn’t complete without labs completed.
